RESIDENTIAL SERVICES AT HORIZON CARE:
Horizon Care Limited delivers individual packages of educational support, clinical support and social and emotional support to young people with complex needs.

We specialise in caring for young people who are behaviourally challenging and emotionally vulnerable, who cannot cope in other types of provision. Our ethos is based on a holistic approach. Each young person has their own key worker, qualified teacher and clinician. We use behaviour management strategies that focus on the avoidance of confrontation and consistent use of risk assessed, planned interventions. Our clinician works with care staff and teachers to address their perceptions of difficult behaviour, develop confidence, improve awareness and promote consistency. The approach focused not only on behaviour change but also on successful behaviour management. All members of the multidisciplinary team share a collective responsibility for the development, implementation and review of the care plan. Roles and responsibilities are clearly defined and set out in the ‘Horizon Care Limited Working in Partnership’ document. It is our shared goal to promote self-esteem through recognition, achievement and praise, and to assist each young person to develop the coping strategies and skills required to manage difficulties as they arise in an environment that offers the correct balance between care and control, protection and risk. We endeavour to help young people understand and deal with the consequences of their own actions and use a structured education timetable and activity plan to establish daily routines, promote a sense of purpose and direction and to channel energies into positive activity.

ASSESSMENT SERVICES
Horizon Care aims to provide a period of stability to allow young people to reflect on their difficulties and make future plans based upon a comprehensive assessment of their social, emotional, physical and educational needs.

We aim to understand, manage and change the difficult, complex and challenging behaviours of the young people who come into our care by meeting each individual’s needs without creating an institutional environment. The philosophy of the organisation is based upon the principles of normality, social inclusion, rights and responsibilities, choice and unconditional positive regard. 

Key Workers work in collaboration with the in-house clinician and education service to provide a comprehensive assessment for each young person and to structure the care plan to meet each individual’s needs. The work is undertaken at the pace of the young person, and allows them to promote their strengths and lessen their difficulties. We support their vulnerabilities and attempt to turn these around and create a resistance to the pulls of their challenging behaviours.

The initial assessment is usually completed within 28 days. At the end of this period comprehensive reports are compiled by the Clinician, the Teacher and the Key Worker. The assessment informs the care plan, the education inclusion plan and the behaviour management plan. The multi-disciplinary team meets every six weeks to review progress and to update the objectives and action plan.
